We look at the whole picture.

Your T1Direct care team regularly reviews how things are going—patterns over time, insulin needs, current settings, recent changes, and the context you've shared.

Then we tell you what we see.

Example T1Direct review
SEPTEMBER 12No changes needed
  • What we noticed

Total daily insulin is up substantially, but the current settings appear to be handling it and lows haven't increased.

  • What we recommend

    Nothing to change for now.

  • What we're watching

We'll keep an eye on breakfast, where corrections have become more frequent.

A NOTE FROM YOUR CARE TEAM

Overall things look good. It's normal to see insulin needs increase at this stage, and your current settings are keeping up well. I'll keep watching the breakfast pattern and let you know if I see anything else.

  • What happens next

We'll check in again in 3 days to see if you're having to intervene less and whether lows have increased.

A NOTE FROM YOUR CARE TEAM

We can see you're working really hard right now to keep the numbers looking good. We'd like your settings to do more of that work for you. Let's make some adjustments and see if we can get you there.

Sometimes something needs to change. Sometimes it doesn't. Either way, we keep looking.

Your plate is already full.

We know you're a diabetes expert. You notice patterns, make adjustments, and learn what works.

But managing diabetes every day and stepping back to assess the trajectory are two very different jobs. Why do we expect families to do both?

You fly the plane. We watch the flight path.

Care that doesn't wait for you to ask.

Every two weeks, we take a fresh look—whether you have a question or not.

And you don't have to keep a running list in your head. Send us an FYI anytime, and we'll have the context when we review.

This isn't urgent, but it feels like we've been correcting a lot more after dinner lately. It's not every night, so I'm not sure if it's actually a pattern.

I keep wondering if his ratio is off, or maybe his target needs adjusting — or if we should just wait and watch. Can you take a look at this at our next review?

A diabetes team that knows your child.

A diabetes expert (CDCES) and pediatric endocrinologist follow your child over time—what's changed, what you've tried, and what we're already watching.

Built from both sides of care.

For nearly 20 years, Lindsey McIlvena, MD, MPH, a physician specializing in Preventive Medicine & Public Health, has built her career finding better ways to deliver healthcare—including more than a decade at the forefront of telemedicine.

Then her 5-year-old son was diagnosed with type 1 diabetes.

T1Direct grew out of the gap in care her family is living through every day—and a career spent reimagining how care can work.

FAQ

Does T1Direct replace our endocrinologist?

No. We add to the team you already have. Keep your regular endocrinology visits and the diabetes team you know. T1Direct adds regular review and follow-up in between.

What if we need you to look sooner?

Just ask. You can request an earlier review anytime. T1Direct isn't continuous or emergency monitoring, so urgent concerns should still follow your existing diabetes team's guidance.

What happens if you recommend a change?

We keep following it. After a meaningful change, we check back to see how it worked—and whether another course correction is needed.

What if everything looks good?

We'll tell you that, too. We see what's working, and we see the work it takes to get there. Keep going—we're with you.
